How it is calculated

Glycated hemoglobin reflects average glucose over 8–12 weeks — the lifespan of a red blood cell. The A1c-Derived Average Glucose study (ADAG, Nathan 2008) matched HbA1c against continuous glucose monitoring in 507 people and derived a linear relationship: eAG (mg/dL) = 28.7 × HbA1c − 46.7. The calculator works both ways — from HbA1c to average glucose and from a known average (e.g. from a meter or CGM) to the expected HbA1c — and converts NGSP percent to IFCC units (mmol/mol) used in Europe and Australia.

Limits of the method

HbA1c is inaccurate in conditions that alter red-cell lifespan or hemoglobin structure: anemia, hemoglobinopathies, pregnancy, CKD, recent blood loss or transfusion, iron and B12 deficiency. In 10–15% of people the individual HbA1c–glucose relationship differs noticeably from the average (the "glycation gap"), so eAG is a population estimate, not a measurement. A diabetes diagnosis requires confirmation by a repeat test.

Compare eAG with your meter readings

If your meter average is noticeably below eAG, you are probably testing mostly fasting and missing post-meal peaks. A gap over 1.5 mmol/L is worth discussing with a physician.

The HbA1c target is individual — from 6.5% to 8.0% depending on age and hypoglycemia risk.

What HbA1c actually measures

Glucose binds irreversibly to red-cell hemoglobin, and the share of such "sugared" hemoglobin grows in proportion to average glycemia. Because a red cell lives about 120 days, HbA1c is a weighted average: the last month contributes about 50% of the value, the second 25%, the third and fourth the remaining quarter. Diet changes therefore show in HbA1c within 4–6 weeks and fully after 3 months.

HbA1c vs fasting glucose and OGTT

Fasting glucose shows one point, the OGTT shows the response to a load, HbA1c shows the average background over a quarter. They identify different people: up to a third of prediabetes cases by OGTT have a normal HbA1c, and vice versa. The ADA accepts any of the tests for diagnosis but, when they disagree, recommends repeating the one that fell in the diabetes range. For treatment monitoring HbA1c is best combined with CGM time in range.

Frequently asked questions

What is a normal HbA1c?

By ADA and WHO criteria normal is below 5.7% (39 mmol/mol). 5.7–6.4% (39–47 mmol/mol) is prediabetes; 6.5% (48 mmol/mol) and above is diabetes when confirmed by a repeat test.

How do I convert HbA1c from percent to mmol/mol?

With the NGSP/IFCC master equation: mmol/mol = (% − 2.15) × 10.929. For example, 6.0% = 42 mmol/mol, 7.0% = 53 mmol/mol. Reverse: % = mmol/mol / 10.929 + 2.15.

Why does my meter average not match eAG?

A meter usually captures selected points, mostly fasting, and misses post-meal and overnight peaks. Also, some people’s red cells live longer or shorter than average, shifting HbA1c. A gap of up to 1 mmol/L is common.

How fast does HbA1c fall after changing diet?

First changes appear within 4 weeks, and a fully new level settles after 8–12 weeks. A 0.5–1.0% drop over 3 months from diet and activity changes is a realistic result without medication.

When is HbA1c unreliable?

In iron-deficiency and hemolytic anemia, hemoglobinopathies (sickle cell, thalassemia), pregnancy, CKD stages 4–5, after blood loss or transfusion, and with high-dose vitamin C and E. Fructosamine, glycated albumin or CGM are used instead.
