## What it is
Selenium is a trace element required for selenoproteins. They help protect cells from oxidative stress, regulate thyroid-hormone metabolism, support immune responses and tissue repair. Requirements are measured in micrograms, so more is not automatically better.

## Food sources
Fish, seafood, meat, eggs, dairy, legumes and whole grains provide selenium. Plant content depends on soil and region. Brazil nuts are highly concentrated and vary widely; eating handfuls every day can create excessive intake.

## Deficiency
Low intake may occur with severe dietary restriction, malabsorption, long-term parenteral nutrition or selenium-poor soils. Severe deficiency is uncommon. Fatigue, hair loss, infections and muscle weakness are non-specific and need a broader assessment.

## Thyroid
Selenium enzymes help convert T4 to active T3 and protect thyroid tissue from peroxides. This does not mean selenium treats every thyroid disorder. It does not replace levothyroxine or justify self-directed attempts to change TSH.

## Testing
Plasma or serum selenium can be measured, but reference ranges depend on method and laboratory. Routine testing is usually unnecessary in healthy people. TSH and free T4 assess thyroid function but do not directly measure selenium stores. Tell the laboratory about supplements.

## Supplements
Products contain selenomethionine, selenocysteine or inorganic salts. Evidence of benefit depends on baseline status. Avoid stacking multiple products and stay within label directions; hair and thyroid formulas often duplicate ingredients.

## Toxicity
Chronic excess may cause garlic breath, metallic taste, nausea, diarrhoea, brittle nails, hair loss, rash and neurological changes. Very large doses can be acutely dangerous. Severe vomiting, confusion or weakness after an overdose requires urgent care.

## Medicines and conditions
Universal interactions are limited, but supplements can confuse symptom assessment. Do not replace thyroid treatment with selenium. Discuss use with a clinician in kidney disease, pregnancy, cancer treatment or polypharmacy.

## Pregnancy
Selenium supports fetal development, but preventive megadoses are not proven safe. Use a varied diet; a supplement decision should reflect diet and clinical indications. Brazil nuts are not a predictable medicine.

## Vegan diets
Review legumes, whole grains, fortified foods and local soil conditions. Do not diagnose deficiency from one symptom. A clinician may assess selenium alongside iron, B12, zinc and thyroid function when restriction is prolonged.

## Practical sequence
1. List dietary sources and every supplement. 2. Remove duplicate formulas. 3. For symptoms, assess common causes and TSH with free T4. 4. Order selenium testing for a defined risk, not routine screening. 5. Avoid megadoses and daily handfuls of nuts. 6. Review the plan with a clinician.
